Cannondale frame and shifter cable housing stop. The housing stop was bolted to the frame using a multi-part assembly including bolts, threaded rod, and intermediates threaded on the inside to engage the threaded rod and bolts.
One of the intermediates failed and the housing stop came detatched from the frame, releasing cable tension. The rider was thrown over the handlebars.
