No specifics on the failure, rider weight, or typical use.

Failures are sometimes started by too much pinch clamp force. Bolt torque is sometimes specified for carbon fiber posts. However, carbon posts seem to be more sensitive to clamp design than metal posts, so following torque specifications may be insufficient.

Note scoring on post, unclear if that contributed to the failure.

my carbon fiber seatpost broke off, at the frame, while seated and going over a bump (actually a pot-hole). Fortunately, I caught myself as soon as I had the falling sensation, and fortunately there were no sharp protrusions, [...]

Czech triathlete and Olympic bronze medallist, Jan Rehula, suffered one of the worst possible consequences of a seat breakage after a training accident in Sydney recently. 27 year old Rehula hit a pothole, snapped his seat, was gouged in the rear by his seat post and lost two litres of blood.

He has so far had two operations and has internal damage in his lower back. His start in the Hawaii Ironman triathlon in October is in doubt, but his spokesman said that he was quite positive that he will be able to resume his career.
