Quill stem failure at weld between quill and extension. Brand unknown
Reportedly:
[I]t had a crack on one side and failed when he stood up.
Unclear if the rider noticed the crack before the final failure.
No specifics on rider weight or typical use.
Dark oxidized aluminum at the bottom and light aluminum at the top indicates the failure started at the bottom and went up. The major repeated load of the lower part is probably compression due to rider weight resting on the handlebar, whereas pulling when e.g., jumping or climbing hills is probably lower force and much lower repetition.
