1999 Sepcialized Allez Sport; failed 2006/06. Aluminum. Used with Shimano Ultegra 9-speed hub. Typically ridden 3,000-5,000 km/yr. Bicycle+rider+gear approx. 125kgf. Typical riding includes hills; climbing is spinning, not standing. Failure discovered via clicking sound.

Dark spot in crack at edge of quick-release nut is screw holding removable dropout hangar.

I didn't have the receipt, from a shop (by then out of business) in a different state. The response from Specialized was completely satisfactory: They replaced the frame under lifetime warranty with a current model (actually an Elite, the next model up, because they had no identical replacements on the shelf) no questions asked. The only way it could possibly have been better might have been if the factory had paid the dealer (Cupertino Bike) to build my new bike for me, but labor isn't part of the terms of their warranty and I didn't feel any need to push for it.
