Note the failure occured near the spindle end of the vanity groove; such failures are typically near the pedal eye.

Appears to be the first Pro 5 vs. model (early 1960s to early 1970s), which looks narrower around the vanity groove than second/third models. "Narrower" means the most-stressed part suffers higher stress.
